Acute Upper Gastrointestinal Hemorrhage in Older People: A Prospective Study in Two Neighboring Districts

Abstract: Older people with acute UGIH have advanced upper gut pathology with preponderance of esophageal lesions. Classical symptoms seem lacking, but mortality can be decreased despite adverse comorbid factors. Lower thresholds for endoscopy are advocated in older adults, and comparative studies of UGIH symptoms with younger patients are required.

“…In this study, we focused on elderly patients with acute UGIB, in terms of mortality and factors predictive of mortality. The cause of UGIB was peptic ulcer bleeding in 84.8% of patients, which is consistent with previous studies [13][14][15][16]. The cause of peptic ulcer bleeding might be from ulcerogenic medications (including aspirin, clopidogrel, NSAIDs, and warfarin).…”

“…Medical conditions requiring diagnostic endoscopy such as anemia, dyspeptic symptoms, acute upper gastrointestinal hemorrhage, or suspicion of malignancy are particularly frequent in older persons 1–3 . Whereas in the past older patients were often not investigated, nowadays endoscopy is increasingly carried out in such patients, because upper gastrointestinal endoscopy is generally considered a safe procedure in older people 4 .…”
