Adaptation and Fidelity: a Recipe Analogy for Achieving Both in Population Scale Implementation

Abstract: Balancing adherence to fidelity of evidence-based programs and adaptation to local context is one of the key debates in the adoption and implementation of effective programs. Concern about maintaining fidelity to achieve outcomes can result in replication of research-based models that can be a poor fit with the real world. Equally, unplanned adaptation can result in program drift away from the core elements needed to achieve outcomes. To support implementation of the Maternal Early Childhood Sustained Home-vis… Show more

“…While the use of evidence‐based programs has become the standard for practice, important questions exist about how to balance fidelity to the model while simultaneously adapting evidence‐based programs to match the contextual needs of a specific community (Backer, ; Berkel et al, ; Kemp, ). Fidelity is defined as the degree to which the core components specified in the original intervention are followed during the implementation of a prescribed program (Berkel et al, ; Durlak & Dupree, ).…”

“…Adaptation has been defined as modifications “to address differences between the context in which the intervention was originally designed and tested, and the one into which it is ultimately implemented” [38]. While fidelity-consistent adaptation is possible [38,39], it has been argued that one can come at a cost to the other [40]. Models adopted from treatment development research are more likely to narrowly define fidelity as adherence to a specific treatment protocol, while increasingly, implementation models consider adaptation a natural and perhaps even essential part of the implementation process [18,41].…”

“…Despite its promise, fidelity to, and/or adaptation of, the PCMH model may impact efforts in facilitating health care and population health improvements. Fidelity to the PCMH model and adaptation to suit a particular context are often conceptualized as competing demands . The tension between intervention fidelity and adaptation is a critical consideration for PCMH implementation and sustainment.…”

“…Fidelity to the PCMH model and adaptation to suit a particular context are often conceptualized as competing demands. [19][20][21] The tension between intervention fidelity and adaptation is a critical consideration for PCMH implementation and sustainment. While seven core principles of the PCMH and a recognition process for meeting nationally recognized PCMH standards have been established, considerable variability exists between PCMH practice transformation and implementation efforts.…”
