Adaptation of a Personalized Electronic Care Planning Tool for Cancer Follow-up Care: Formative Study

Abstract: Background Most patients diagnosed with colorectal cancer will survive for at least 5 years; thus, engaging patients to optimize their health will likely improve outcomes.
