Added value of diffusion weighted MRI in the diagnosis of postpartum ovarian vein thrombosis

Abstract: A previously well 31-year-old woman who had given birth to a healthy baby eleven days previously, presented with a undulating fever and nausea that started immediately in the postpartal period after an uncomplicated vaginal delivery. Clinical examination revealed an elective tenderness in the right fossa without rebound tenderness. Rovsing's sign, Murphy's sign, Murphy's punch sign and the psoas sign were all negative.

“…MRA offers several advantages over CT in diagnosis of OVT. It avoids ionizing radiation, has sensitivity and specificity approaching 100% compared to CT with sensitivity of 77.8% and specificity of 62.5%, and avoids intravenous contrast administration due to the use of diffusion weighted imaging [ 7 , 9 , 10 ]. Despite these benefits, many physicians use CT as the first imaging tool due to its easy availability.…”

“…MRI involves multiple planes and, because of increased sensitivity of paramagnetic iron, it can differentiate between flowing blood, acute thrombus (less than 1 week old) and subacute thrombus (1 week to 1 month old) . Although MRI does not require contrast and does not have ionising radiation risks, it is not as readily available as CT in most units …”
