Addition of doxycycline to fluoroquinolones for bacterial prophylaxis in autologous stem cell transplantation for multiple myeloma.

Abstract: e18732 Background: Autologous stem cell transplant (ASCT) is standard therapy for patients (pts) with multiple myeloma (MM), yet the risks of such treatment include neutropenic fever (NF) and bacteremia. Fluoroquinolones (FQs) are a mainstay of bacterial prophylaxis (ppx) of NF in ASCT at many transplant centers and reduce the incidence of bacteremia. Nevertheless, ppx regimens for ASCT are not standardized and variability exists even within single institutions. While antibacterial ppx decreases infections, a… Show more

“…In this issue, Guare et al. provided a detailed exploration of the impact of antibiotic prophylaxis in a large cohort of multiple myeloma (MM) autologous stem cell transplant (ASCT) recipients, who received standard‐of‐care conditioning 3 . Their central question was whether broader antibacterial prophylaxis, through the addition of doxycycline to ciprofloxacin or levofloxacin, would improve patient outcomes compared to FQ monotherapy.…”

“…In this issue, Guare et al provided a detailed exploration of the impact of antibiotic prophylaxis in a large cohort of multiple myeloma (MM) autologous stem cell transplant (ASCT) recipients, who received standard-of-care conditioning. 3 Their central question was whether broader antibacterial prophylaxis, through the addition of doxycycline to ciprofloxacin or levofloxacin, would improve patient outcomes compared to FQ monotherapy. Their findings are worthy of contextualization within the recent body of high-quality randomized-controlled trials (RCTs) and observational cohort studies that have re-examined the impact of antibacterial prophylaxis on outcomes in neutropenic patients, including those with MM.…”
