Adenocarcinoma arising at ileostomy sites: Two cases and a review of the literature

Abstract: Total colectomy with ileostomy placement is a treatment for patients with inflammatory bowel disease or familial adenomatous polyposis (FAP). A rare and late complication of this treatment is carcinoma arising at the ileostomy site. We describe two such cases: a 78-year-old male 30 years after subtotal colectomy and ileostomy for FAP, and an 85-year-old male 50 years after colectomy and ileostomy for ulcerative colitis. The long latency period between creation of the ileostomies and development of carcinoma su… Show more

“…It has been hypothesized that colonic metaplasia occurs when the intestinal mucosa of an ileostomy is exposed to chronic inflammation, creating a cycle of mucosal damage and repair. This could occur through trauma, poorly fitting or irritating stomal appliances, or other changes in the environment of the ileostomy . How colonic metaplasia becomes intraepidermal is unknown.…”

“…The first reported case of primary adenocarcinoma arising from an ileostomy site occurred in 1969 in a patient with longstanding UC . Since then, approximately 42 cases of ileostomy site primary adenocarcinoma have been reported in the literature in individuals with IBD or familial adenomatous polyposis . These malignant changes only arose after decades of stoma use.…”

“…Similarly, adenocarcinoma arising at an ileostomy site is a rare complication after colectomy and ileostomy formation for inflammatory bowel disease (ulcerative colitis, Crohn's disease) or familial adenomatous polyposis (FAP) [6][7][8][9]. The precise etiology of ileostomy carcinomas remains unclear, but colonic metaplasia from chronic inflammation has been considered to be correlated.…”

“…According to the literature, ileostomy adenocarcinomas usually demonstrate fungating, exophytic growths involving the mucosa of the ileostomy [6][7][8][9]. Clinically, they can appear in various forms such as chronic ulcers or protruding masses.…”
