Adenocarcinoma in Solitary Large Hyperplastic Polyp Diagnosed by Magnifying Colonoscope

Abstract: We report a case of carcinoma in a hyperplastic polyp in a 78-year-old female that was diagnosed before resection using a magnifying colonoscope. The patient presented with fecal occult blood and underwent total colonoscopy, which revealed a 12-mm sessile polyp in the cecum. When seen in magnified view, an irregularly shaped pit was evident at the center of the polyp that was distinct from the asteroid-type pits observed over most of the lesion. We diagnosed this lesion as a hyperplastic polyp with a carcinoma… Show more

“…We routinely use a magnifying colonoscope because the insertion technique and manipulation are similar to those for an ordinary colonoscope [8]. In a prospective study, Konishi and colleagues reported that insertion of a magnifying colonoscope into the cecum was achieved successfully in 97% of cases, and that there were no significant differences in the average time taken to reach the cecum or the average total procedure time [9].…”

“…Some reports currently describe large HPs as occurring on the right-side 3 in association with synchronous colorectal cancer (CRC), and showing a malignant potential. [4][5][6] However, a few investigators have reported that intermediate size HPs (≥ 6 mm) also have neoplastic potential. 7,8 It is accepted as beyond doubt that intermediate size HPs become large HPs (≥ 10 mm) in time and should not be lumped together with the common variety of diminutive HPs.…”
