Adenoid cystic carcinoma of the lacrimal sac: case report

Abstract: Lacrimal sac tumors are rare with a clinical presentation that typically includes obstruction of the lacrimal drainage system and epiphora as the most frequent symptom. Cribriform adenoid cystic carcinoma (ACC) is the most common malignant epithelial tumor of the lacrimal gland and minor salivary glands; however, its occurrence in the lacrimal drainage apparatus is extremely rare. Given the rarity of ACC, definitive diagnosis is almost invariably late conferring a poor prognosis. Herein we report the case of a… Show more

“…The most appropriate management of lacrimal sac tumors depends more on tumor size and the general condition of the patient. Exenteration does not ensure a better prognosis and causes substantial facial disfigurement which may cause tremendous physiological and psychological trauma to the patients . In our study, only 37.2% of the patients (19/51) selected extended resection, two underwent orbital exenteration, and 56.9% of cases (29/51) received conservative resection, which was different from traditional management.…”

“…Exenteration does not ensure a better prognosis and causes substantial facial disfigurement which may cause tremendous physiological and psychological trauma to the patients. 14,15 In our study, only 37.2% of the patients (19/51) selected extended resection, two underwent orbital exenteration, and 56.9% of cases (29/51) received conservative resection, which was different from traditional management. One possible reason may be that radiotherapy has played an extremely important role in the treatment of lacrimal sac SCC.…”

“…He was diagnosed and treated as chronic dacryocystitis at the local hospital, but the symptoms were not improved. Therefore, we need to be alert to the possibility of malignant lacrimal sac tumors in patients with chronic dacryocystitis who had poor respond to standard treatments (11).…”

“…Complete excision of the tumor and lacrimal drainage system followed by radiotherapy with or without adjuvant chemotherapy was still the main treatment for ACC of the orbit (16)(17)(18). However, there are also argues that surgery in this area often involves orbital exenteration that would substantially impair the quality of life and raise psychological and psychiatric disorders in patients (11,19), while the prognosis might not necessary be improved (20). Therefore, extensive surgery should be carefully evaluated for patients with ACC of the lacrimal sac, especially in young patients such as that in the present case.…”

“…The case was a 41-year-old woman with late diagnosis, who underwent ophthalmological examination and multidisciplinary treatment, and was finally confirmed as primary ACC. [13] The ACC also occurred in the female reproductive system. For instance, a 23-year-old woman was diagnosed ACC in left ovary, the tumor had the typical cribriform pattern of ACC, lacked any component of surface epithelial carcinoma, and showed myoepithelial differentiation.…”
