Adenoid Cystic Carcinoma of the External Auditory Canal

Abstract: ACC of the EAC often recurs many years after definitive treatment. Although our sample size was too small to make definitive conclusions, we recommend aggressive local therapy with lateral temporal bone resection and adjuvant postoperative radiotherapy. In addition to successful local therapy, early diagnosis may be the only other effective means of preventing distant metastases.

“…Adenoid cystic carcinoma (ACC) in the minor salivary glands, including the larynx, nasal cavity, paranasal sinus, external auditory canal and trachea, is uncommon (1)(2)(3)(4). ACC is noted for its slow growth and indolent course and, as with ACC in the major salivary glands, patients with ACC of these sites often experience recurrence and distant metastases many years after diagnosis and definitive treatment (5).…”

“…Those of glandular origin comprise just 20% of EAC tumors, with adenoid cystic carcinoma (ACC) of EAC being extremely rare [1]. Adenocystic carcinoma of the head and neck is usually found in the salivary glands, oral cavity, nasopharynx, and palate [2]. There have been only a few cases of adenocystic carcinoma involving the EAC reported in the international literature.…”

“…A BCC typically shows spindle-shaped nuclei, whereas an AdCC has rounded nuclei with a scarcity of mitotic figures [1] . As with most AdCCs, those that originate from the ceruminous glands also show perineural invasion [3] .…”

“…This significantly delays the diagnosis and hence the survival rate as a cutaneous neoplasm is not seen. The median time from the onset of symptoms to diagnosis has been reported to be around 2 years [3] . The rate of local recurrence and distant metastasis reaches more 40% and 21%, respectively, despite lateral temporal bone resection [3] .…”
