Adenovirus infection and disease in recipients of hematopoietic cell transplantation

Abstract: Purpose of review To provide an update on risk factors associated with adenovirus (ADV) infection in patients after hematopoietic cell transplant (HCT) and on options for ADV monitoring and treatment in the setting of HCT. Recent findings Among patients undergoing HCT, ADV infection continues to be more common amongst those receiving a T-cell-depleted or graft other than from a matched-related donor. Among children undergoing HCT, reactivation in the ga… Show more

“…Nephrotoxicity from cidofovir is an important concern and can be worsened with the concomitant administration of some common antimicrobial and immunosuppressive medications. 26 We noted a higher incidence of renal dysfunction and CRRT utilization in the cohort of cases receiving cidofovir.…”

“…24,25 There is currently no approved antiviral treatment for adenovirus in North America or Europe though cidofovir is routinely used as a preemptive therapy in HSCT patients and we noted widespread use among our immunocompromised cases. 26 It was notable that 25% of immunocompetent cases received cidofovir despite a relatively low rate of adenoviremia in this group. Nephrotoxicity from cidofovir is an important concern and can be worsened with the concomitant administration of some common antimicrobial and immunosuppressive medications.…”

“…Considering that no anti-HAdV drugs are presently in clinical use, disinfectants are important for preventing outbreak of HAdV (1,10) and it is important to evaluate the virucidal effects of disinfectants on HAdVs.…”

“…HAdV-B11 associated with hemorrhagic cystitis is a major concern in hematopoietic stem cell transplantation because it causes secondary infection due to the infection of the virus in the environment (6)(7)(8)(9). Post transplantation infection caused by adenovirus species B or C, is a problem worldwide (10)(11)(12)(13)(14)(15)(16). Adenoviruses associated with these infections are known to be explosively transmitted in confined spaces such as hospitals.…”
