Are patients who self-administer their medicines in hospital more satis®ed with their care? Patient self-administration of medicines in hospital prior to discharge is being seen increasingly as good practice by health professionals. Previous studies have looked for increased compliance and knowledge or asked whether patients liked self-administration and have not clearly demonstrated the bene®ts. This study surveys patients' views on self-administration and on their care. In particular it looks at the discharge process and the way information was given to the patient on discharge. Questionnaires were distributed to 309 patients being discharged from general medical wards of a teaching hospital in Central England to be completed by the patients after their discharge from hospital. Of these, 202 were returned to a separate university department for analysis. Although this study has been undertaken on only two wards, the survey has potential for large scale use. Findings which were consistent with previous work were that a great majority of patients who had self-administered their medicines would like to again, whilst those who had not had that opportunity were less likely to choose to self-administer in the future. However, this study found that a majority of patients under 60-years-old would choose to selfadminister their medicines in hospital even if they had not been given the opportunity to do so recently. It was also found that patients who had administered their own medicines in hospital were more likely to report their overall care as excellent and were more satis®ed with the discharge process than patients who had not.

This study compares the knowledge and attitudes of 60 patients in a state hospital about medication, before and after structured and unstructured medication education. The purpose was to determine which type of education has more positive effects. A pre- and post-test two-group experimental design was used. The results of this study were not statistically significant in supporting the literature that education has some effect on knowledge or on attitude about taking medication. The study showed similar effects of the method (structured versus unstructured) on increasing knowledge and attitude toward medication. Demographic data from the study suggest a variety of other factors may relate to attitudes and knowledge about medication in the hospital. This study suggests medication education should address demographic factors, incorporate multi-dimensional approaches, include more active patient participation, and utilize one-to-one teaching whenever possible.

This paper aims to report on the literature related to self-medication programmes that contributed to the development of an Older Persons Mental Health (OPMH) specific pathway for a self-administration of medication (SAM) programme. The traditional methods used within mental health inpatient environments to administer medication are paternalistic and resonant of the medical approach to care. There is a need to investigate innovative methods in the management of depression in older adults that reflect an individualized, innovative approach to redress the power imbalance for patients. Electronic databases were searched using the search strategy of Hek et al. to identify key themes in the implementation of self-administration programmes. Seven themes emerged from the literature review: the structure of SAM, assessments and risks associated with SAM, patient education, SAM and medication adherence, the patients' perspective, advantages and barriers to SAM, and professional issues. These findings were then used as the foundation for a project to develop an OPMH specific pathway for the implementation of SAM. There is a distinct lack of implementation of SAM programmes across the UK, especially within mental health services. Older Persons Mental Health is ready for the implementation of change in medicines management for older adults. Self-administration of medication can ensure all the principles of NHS plan relating to individualized person-centred care are achieved. Nurses have the opportunity to actively contribute to this process, facilitating and empowering those in their care to be experts within their journey and take control of their illness.
