Adherence to guidelines and suboptimal practice in term breech delivery with perinatal death- a population-based case-control study in Norway

Abstract: Background In a recent population-based study we reported excess risk of neonatal mortality associated with vaginal breech delivery. In this case-control study we examine whether deviations from Norwegian guidelines are more common in breech deliveries resulting in intrapartum or neonatal deaths than in breech deliveries where the offspring survives, and if these deaths are potentially avoidable. Material and methods Case-control stu… Show more

Fifteen studies 12 , 15 , 27 , 33 , 43 , 46 , 48 , 50 , 58 , 60 , 63 , 66 , 70 , 71 out of 68 received funding from two or more organisations (22%), but 24 out of the 68 studies (35%) received no funding or did not list any funding information for the stillbirth review process.…”

“…However, observation from personal experience is often based on small samples and prone to recall and confirmation bias . Reviews of neonatal mortalities associated with term vaginal breech births often identify suboptimal care, and delay is often implicated . Identification of common characteristics of births with good outcomes may provide additional insight.…”

“…25 Suboptimal antenatal care and/or management of in-hospital deliveries is more common in deliveries with intrapartum or neonatal death than in deliveries where the offspring survives. 26 Multiple methods have been tried in the search for improved birth outcomes for breechpresenting fetuses. A retrospective cohort study published in 2017 compared vaginal delivery in the dorsal position with vaginal delivery in an upright position.…”
