Adherence to positive airway pressure therapy

Abstract: Adherence to CPAP treatment is an important clinical issue that needs to be routinely assessed and addressed. Frequent patient contact to troubleshoot problems will contribute to adherence. Further study is needed to identify factors that affect adherence and cost-effective interventions.

“…In the U.S., obesity and OSA are both on the rise. But only about 20% of OSA is diagnosed, and even a smaller percentage is effectively treated [27][28][29][30]. OSA is a well-known independent risk factor for a variety of diseases related to cardiovascular disease, including hypertension, diabetes, cardiac arrhythmia, stroke, and coronary heart disease [5,8,9,14,31].…”

“…a significant impact in promoting adherence compared to educational and technologic enhancements alone. 20,21,24,25,30,31 There are few published data on CPAP therapy initiation and utilization specifically in CMV operators, and no studies report above average adherence; we located only two published studies. In one study of commercial bus drivers in Hong Kong, from a population of 85 drivers with OSA, 25 were referred for evaluation with 9 using CPAP therapy, averaging 4.5 h use after 3 months.…”

“…9 PAP compliance is typically defi ned as a minimum of 4 hours of use per night for at least 70% of the nights, [9][10][11] although some studies indicate that the use of PAP for 6 or more hours per night may be more clearly associated with improvement in sleepiness, daily functioning, and memory. 12 PAP devices commonly incorporate software that measures and records adherence data such as the number of days and duration that the PAP device is used as well as its effi cacy, allowing for more accurate assessment of success with treatment. The impact of treatment on daytime sleepiness can be assessed with the Epworth Sleepiness Scale (ESS), a validated questionnaire that can be administered before and after starting PAP therapy to measure changes in daytime sleepiness.…”
