Adjustment of Insulin Pump Settings in Type 1 Diabetes Management: Advisor Pro Device Compared to Physicians’ Recommendations

Abstract: Aims: To compare insulin dose adjustments made by physicians to those made by an artificial intelligence-based decision support system, the Advisor Pro, in people with type 1 diabetes (T1D) using an insulin pump and self-monitoring blood glucose (SMBG). Methods: This was a multinational, non-interventional study surveying 17 physicians from 11 countries. Each physician was asked to provide insulin dose adjustments for the settings of the pump including basal rate, carbohydrate-to-insulin ratios (CRs), and corr… Show more

“…Thus, as there is a platform where you can see various technologies in one place, it is necessary to develop a decision support tool that can help with complex insulin dose calculating over a short time through that platform for optimal management. Indeed, Glooko has made a digital decision support system called DreaMed Advisor Pro (DreaMed Diabetes Ltd., Petah Tikva, Israel) which automatically provides the exact insulin dosing recommendations and other treatment tips through an AI-based algorithm [ 71 ]. In a study comparing physicians to DreaMed Advisor Pro in insulin dose adjustment, the changes in insulin dosing were similar to those given by the physicians.…”

“…The Endo.Digital DSS (version 2, DreaMed Diabetes Ltd) carries out a similar purpose for users of MDI. While the majority of studies conducted by Nimri et al were non-interventional and compared insulin dose adjustments using these DSSs with those made by physicians using retrospective data, [37][38][39] one study evaluated real-time remote insulin adjustments by the DreamMed Advisor Pro to physicians every 3 weeks over a 6-month period, demonstrating similar levels of glycaemic control in children and adolescents using a pump and CGM. 40 However, one limitation of this system is the intention to help clinicians with therapy adjustments, instead of providing them directly to the patient.…”

“…Along with the prediction, the algorithm shows the systolic and diastolic phases considered; however, the specifics of the predictions “are protected under proprietary regulations,” ( 22 ) leading to inclusion of this algorithm in the translucent category. Likewise, the DreaMed algorithm from DreaMed Diabetes, Ltd., produces reports with important features related to its insulin dose recommendations ( 23 , 24 ), placing it in the translucent category.…”
