Adjuvant cytotoxic liver perfusion for colorectal cancer

Abstract: Colorectal liver metastases develop by malignant cells entering the portal venous circulation. A randomized prospective clinical trial was commenced in 1975 to assess the value of adjuvant umbilical vein cytotoxic perfusion (with 5-fluorouracil) following colorectal resection. One hundred and fifty-four patients without macroscopic liver secondaries have so far entered the trial. The mean age, sex, site and stage of the disease were similar in the control and perfusion groups and there was no statistically sig… Show more

“…Use of the portal vein appears advisable for small liver metastases (Ackerman et al, 1969). Immediate post-surgery infusion of 5-flourouracil (5-FU) in the portal vein has recently been shown to significantly reduce the rate of disease recurrence at two years for Dukes' C lesions (Taylor et al, 1979). Ensminger et al (1978) have published detailed pharmacokinetic data concerning the i.a.h.…”

“…Whereas a longer course might have revealed some effect, the intention of this study was to examine the effect of a short course equivalent to that used in adjuvant liver perfusion in man (Taylor et al 1979). From this viewpoint, any concern that such therapy might promote further large bowel neoplasms would thus seem to be groundless.…”

“…Thus, effective adjuvant treatment is essential to prevent liver metastasis. Many efforts have been attempted in this setting, adjuvant portal venous chemotherapy is an option to prevent the development of colorectal liver metastasis with some encouraging results (Taylor et al, 1979). Taylor et al further suggested that 5-fluorouracil (5-FU) could be effective as an adjuvant therapy if infused via the portal vein (PVI) (Taylor et al, 1985).…”
