Adjuvant therapy of biliary tract cancers

Abstract: Biliary tract cancers (BTCs) are rare and heterogeneous malignant tumours including cholangiocarcinoma and gallbladder cancer. They are very aggressive, often refractory to chemotherapy and associated with an overall poor prognosis. Surgical resection remains the only potentially curative treatment option but less than 35% present with resectable disease. Adjuvant treatments have been widely used but until recently, supportive data were limited to non-randomised, non-controlled retrospective studies. Recent ev… Show more

“…While the surgical resection of BTCs, when feasible, represents the primary curative modality for managing these diseases, the rates of post-operative recurrence are unacceptably high at 50–70% [ 112 ]. The current standard of care for resectable BTCs is adjuvant capecitabine, based on the phase III BILCAP study that compared capecitabine to surveillance.…”

“…Besides liver transplantation, radical surgical resection remains the only curative therapeutic option, although recurrences are very common. Consequently, adjuvant therapeutic concepts have been established in cholangiocarcinoma (CCA) and are recommended by international guidelines [6]. Adjuvant treatment after resection of HCC is also being investigated but is not the current standard of care.…”

“…Surgical resection remains the sole curative therapeutic option, albeit with frequent recurrences. Consequently, adjuvant therapeutic concepts have been established and recommended by international guidelines for CCA treatment [8]. Early symptoms of CCA are often absent, leading to late-stage diagnoses where surgical resection is not an option.…”
