Adrenocortical carcinoma: is the surgical approach a risk factor of peritoneal carcinomatosis?

Abstract: Context: Peritoneal carcinomatosis (PC) is a rare site of distant metastases in patients with adrenocortical cancer (ACC). One preliminary study suggests an increased risk of PC after laparoscopic adrenalectomy (LA) for ACC. Objective: The objective of the study was to search for risk factors of PC including surgical approach. Conclusion:We found an increased risk of PC after LA for ACC. Whether this is related to an inappropriate surgical approach or to insufficient experience in ACC surgery should be clarif… Show more

“…Later on, the same group published their results when they did a head-to-head comparison of laparoscopic and robotic PRA on 31 patients each. Although the mean (standard error of the mean, SEM) OR time was similar in both the groups, there was a significant difference in the mean (SEM) OR time of the last 21 cases of robotic-assisted PRA when compared to the laparoscopic group (139.1 [10.9] [40][41][42][43][44] Robotic adrenalectomy has so far been mainly performed for benign diseases, but it has also been reported for adrenal cancer, adrenal metastasis and oncocytoma. “…Three recent studies have reported a higher risk of peritoneal carcinomatosis associated with laparoscopic resection vs open adrenalectomy in ACC surgery (22)(23)(24). However, all these studies were retrospective in nature and included few patients with laparoscopic approach.…”

“…Bununla birlikte, laparoskopik cerrahi sonrası peritoneal nüks ve karsinomatozis gelişiminin açık adrenalektomiye göre daha fazla saptanması nedeniyle, genel yaklaşım adrenokortikal karsinomda açık adrenalektominin yapılmasıdır [10,11].…”
