Adult proximal humerus locking plate for fixation of paediatric subtrochanteric fractures

Abstract: Internal fixation of paediatric subtrochanteric fractures using the proximal humeral locking plate of the ipsilateral side appears to be a good treatment option for the age group of 10-16 years, though comparative studies with a larger sample size are required to further support this observation.

“…Few reports exist in the literature regarding the use of adult proximal humerus plates for pediatric subtrochanteric femur fractures. To the best of our knowledge, Gogna et al reported the largest series including 8 pediatric subtrochanteric femur fractures treated with adult proximal humerus plate and showed favorable outcomes 13. Chew et al used the adult proximal humerus plate for the treatment of a subtrochanteric femur fracture in a 13-year-old female patient and reported a satisfactory outcome 14.…”

“…Previously, adult proximal humerus locking plates 5 , 6 and adult lower-extremity periarticular locking plates 9 have been used for pediatric subtrochanteric fractures. Since these are monoaxial locking plates, it is impossible to adjust the direction of screw insertion.…”

[1][2][3] There are various deforming forces around this fracture like proximal fracture tends to flex, abduct, and external rotate and distal fragment adducts.…”
