Adult Wilms tumor with inferior vena cava thrombus and distal deep vein thrombosis – a case report and literature review

Abstract: BackgroundAdult Wilms tumor (WT, nephroblastoma) is a rare, but well-described renal neoplasm. Although inferior vena cava tumor thrombosis is present in up to 10% of Wilms tumors in childhood, only few cases of this clinical manifestation in adults have been reported. To the best of our knowledge, this is the first case of adult WT infiltrating into inferior vena cava (IVC) with concomitant distal deep vein thrombosis.Case presentationA 28-year-old male patient with gross hematuria and right flank pain was di… Show more

“…Around 300 adult Wilms' tumor cases have been reported in the literature [4]; due to the scarcity of cases, no clinical studies reporting a standard of management in the adult age are available, and in most isolated cases, the management is extrapolated from pediatric guidelines [6][7][8][9][10][11][12][13]. In the particular case of Colombia, only two adult Wilms' tumor cases have been reported, which were in an initial stage and underwent radical nephrectomy [7,8].…”

“…In inferior vena cava, tumor thrombus is observed in 4-10% of patients, with 1-3% of right atrium involvement. 17 Therefore, preoperative cardiac examinations should be performed to prevent cardiovascular complication. This report can help pediatric surgeons become more familiar with cardiomyopathy caused by WT.…”

“…However, anticoagulation should be considered if a plain thrombus is found in addition to a tumor thrombus or if distal thrombosis develops without collateral venous pathway complicated with deep venous thrombosis (Quencer et al, 2017;Ratajczyk et al, 2018). Continuously monitoring vital signs will detect early signs of PE such as desaturation and tachycardia.…”
