В статье рассмотрены современные подходы к терапии артериальной гипертензии у пациентов с сахарным диабетом, приведены результаты исследований влияния основных групп антигипертензивных препаратов на риск инфицирования SARS-CoV-2 и течение инфекции. Особое внимание уделено вопросам стратификации и коррекции сердечно-сосудистого риска при сахарном диабете, безопасности применения статинов у пациентов с COVID-19 и роли фиксированных комбинаций в повышении приверженности лечению для снижения сердечно-сосудистого риска в период пандемии. The article considers modern approaches to the therapy of arterial hypertension in patients with diabetes mellitus, presents the results of studies of the influence of the main groups of antihypertensive drugs on the risk of SARS-CoV-2 infection and the severity of the course of COVID-19. Particular attention is paid to the stratification and correction of cardiovascular risk in diabetics, the safety of statins in patients with COVID-19 and the role of fixed combinations in increasing adherence to treatment to reduce cardiovascular risk during a pandemic.

“…Важным наблюдением было то, что большинство пациентов, у которых возникли эпизоды гипогликемии, сообщили, что принимали пищу в среднем ≤2 раз в сутки во время Рамадана. Эпизоды гипогликемии возникали в среднем через 8,3±4,7 ч после последнего приема пищи, что подчеркивает важность приема пищи перед рассветом (сухур Исследование ADVANCE-ON продемонстрировало реальную возможность сохранения полученных результатов [20,21]. Высокая значимость результатов исследований ADVANCE и ADVANCE-ON позволила внести дополнения в инструкцию к препарату, а именно, что показанием к назначению гликлазида МВ является «Профилактика осложнений сахарного диабета: снижение риска микрососудистых (нефропатия, ретинопатия) и макрососудистых осложнений (инфаркт миокарда, инсульт) у пациентов с сахарным диабетом 2 типа путем интенсивного гликемического контроля» [22].…”

Sulfonylureas are widely prescribed all over the world, mainly because of their high effectiveness. At the same time, the heterogeneity of the group is obvious, primarily in relation to the effect of drugs on the risk of developing hypoglycemic events. The review presents the results of new studies, including «Comparative effectiveness of gliclazide modified release versus sitagliptin as second-line treatment after metformin monotherapy in patients with uncontrolled type 2 diabetes» and results of a model-based meta-analysis of 24 antihyperglycemic drugs for type 2 diabetes, confirming the higher efficacy of modified release gliclazide (gliclazide MB) compared to DPP-4, as well as a low risk of hypoglycemic conditions. The results of recent studies, as well as the results of the ADVANCE and ADVANCE-ON studies, suggest that gliclazide MB is safe to use as a second-line drug, especially in patients with chronic kidney disease.
