Introductioninternal supravesical hernia (ISH) is an exceptional subtype of internal hernia often presenting with small bowel obstruction (SBO). Its rarity makes preoperative suspicion and diagnosis very difficult in an emergency setting.Methodswe retrospectively analyzed the database of patients admitted in a single center emergency unit for small bowel occlusion (SBO) in virgin abdomen and treated by surgery from August 2013 to October 2018. The patients with intraoperative diagnosis of ISH were included in this study.Resultsfrom 29 patients with virgin abdomen submitted to surgery for SBO, two cases of ISH were recorded. In both cases preoperative diagnosis was made by CT scan and urgent treatment was successfully performed by laparoscopy, reducing the entrapped small bowel and closing the hernia's ring by continuous suture. No intestinal resection was needed.Discussionurgent laparotomic repair of obstructive ISH is the standard treatment although laparoscopic approach has also been described in a small number of cases. We reported our experience on two cases in which totally laparoscopic treatment was successfully performed in patients with stable hemodynamic parameters thanks to early diagnosis and limited intestinal distension. By an extensive analysis of the international literature, clinical, diagnostic and therapeutic aspects of this form of internal hernia were discussed.ConclusionCT scan facilitates early ISH preoperative diagnosis, reducing the risk of small bowel resection and increasing the chances of minimally invasive laparoscopic treatment.

Retroperitoneal pseudocyst (RPC) is an uncommon surgical entity. The pseudocyst is characterized by the absence of epithelial lining in the cyst wall. Mostly, it occurs as a sequela of pancreatitis. Pseudocyst due to a non-pancreatic cause, however, is very rare. We report a 49-year-old male, who presented to the emergency department with moderate intensity lower abdominal pain. Computed tomography scans revealed a huge retroperitoneal cyst which was overlying the right ureter and right iliac vessels. The patient underwent laparoscopic excision of the cyst and recovered without any complications. Histopathological examination showed a non-pancreatic RPC. In conclusion, non-pancreatic RPC is a rare surgical disease which can result in pressure symptoms depending on its location and size. In our patient, it was treated by laparoscopic excision despite its proximity to iliac vessels and ureter. A laparoscopic approach using safe surgical principles is a viable option for non-pancreatic RPC. The proximity of the lesion to the iliac vessels and the ureter can be carefully navigated safely by laparoscopy. The ureter can be confirmed by stimulating peristalsis of the duct when in doubt.

Schwannomas are typically benign tumours of the peripheral nerves. However, they seldom arise from the obturator nerve. Here we report a case of an uncommon swelling (2.5 × 3.5cm) in a 65-year-old male cadaver which was found during the routine dissection for first MBBS students in the department of Anatomy, Kasturba Medical College, Manipal, India in the month ofDecember 2019. It was seen originating from the left obturator nerve in the pelvis at the level of the sacral promontory. Histopathological investigation revealed a schwannoma. The hypocellular tumor was arranged in a sweeping fascicle pattern with patches of myxoid degeneration. Obturator schwannomas, though rarely seen, can exist in the cadaver, as seen in the present case. Hence, it should be considered as a differential diagnosis for clinical cases of pelvic masses and eliminated only after thorough radiological examination. Knowledge about the existence of such schwannomas is, therefore, a must. Keywords: Schwannoma, Obturator nerves, neurilemmoma, nerve sheath neoplasms.
