Adventitial cystic disease of the popliteal artery communicating with the knee joint

Abstract: We describe the case of a patient with adventitial cystic disease of the popliteal artery in which a direct anatomic communication between the cysts and the nearby knee joint was demonstrated by magnetic resonance imaging and confirmed by surgery. This unusual observation could shed some light on the much debated question of the cause, the pathogenesis, and the management of the affection. Moreover, it emphasizes the importance and the role of magnetic resonance imaging in the diagnosis of adventitial cystic d… Show more

“…2 The detection of such communications seems to be important for distinguishing this disease from other types of arterial occlusive diseases. Various modes of treatment have been performed to correct the popliteal arterial flow, i.e., the aspiration of the cystic contents, 9 a resectional adventitial cystotomy, 6 thrombolytic therapy by urokinase infusion, 10 and a resection of the affected artery and bypass grafting.…”

“…Several investigations have succeeded in preoperatively detecting a communication between the cyst and the knee joint using computed tomography (CT) or magnetic resonance imaging (MRI). 1,2 We present a case of cystic disease of the popliteal artery in which a communication between the popliteal artery and the capsule of the knee joint was clearly demonstrated by preoperative MRI. A histologic study compressed the arterial lumen (Fig.…”

“…21,3943 This theory gained support, with authors reporting connections between adventitial cysts and adjacent joint capsule. 4448 Linquette et al 49 proposed “systemic myxomatous degeneration” theory, which failed to get much support, as follow-up in patients with CAD failed to show systemic lesions. 16 More recently, Levien and Benn proposed the “unifying hypothesis” in support of the embryological theory.…”

“…2) Cases of ACD communication to the knee joint have been reported, 3,4) indicative of a synovial or ganglion theory. However, in the present case, there was no communication between the cystic lesion and the knee joint.…”
