Affected Segments of the Median Nerve Detected by Fractionated Nerve Conduction Measurement in Vibration-Induced Neuropathy.

Abstract: Peripheral neuropathy in the hand has often been reported in workers using hand-held vibrating tools. But the affected location in the hand is not clearly demonstrated. To elucidate the impaired segment of the median nerve within the hand, fractionated median sensory nerve conduction velocity (SCV) was measured in the digital, finger-to-palm, palm-to-wrist and wrist-to-elbow segments. Subjects were 56 patients with hand-arm vibration syndrome and 43 healthy controls of similar age. SCV in the digital and the w… Show more

“…Precise control for gripping or picking up objects requires exact sensory information from SAI afferents, and to a lesser extent FAI afferents. HAVS patients often have digital nerve impairments 13,14) and damaged cutaneous perception in vibration, pain, thermal and tactile perceptions. Impaired vibrotactile perception threshold is reportedly associated with slowed digital sensory nerve conduction 15) .…”

“…All studies were performed on a Nicolet Viking Quest 1 device. The approach to SNCV was adapted from that developed by Sakakibara et al [1994Sakakibara et al [ , 1998]. There was a common site of stimulation for all distal measurement of the median and ulnar nerves with recording electrodes placed at the mid-palm, base and distal digit, in order to separate a proximal digit to distal digit measurement (PDDD) from a more conventional wrist to proximal digit (WPD) and wrist to palm (WP) measurement.…”
