Age-related changes of standardized uptake values in the blood pool and liver: a decade-long retrospective study of the outcomes of 2,526 subjects

Abstract: Background: Background activity on fluorine-18-fluorodeoxyglucose ( 18 F-FDG) positron emission tomography/computed tomography (PET/CT) is often used as a reference to assess a patient's response to tumor treatment. To produce a suitable background activity reference, we examined the variations in standardized uptake values (SUVs) in the blood pool and liver of a large multi-aged population. Methods: A total of 2,526 subjects underwent 18 F-FDG PET/CT examinations and were divided into 12 age groups. “…Since aged livers are associated with less blood perfusion and decreased hepatocyte number, one would expect aged livers have less fluorine-18-FDG uptake. However, it has been well documented that aging is associated with an increase of both liver maximum standard uptake value (SUVmax) and liver mean standard uptake value (SUVmean) on FDG positron emission tomography (PET) imaging (11,(70)(71)(72). Cao et al (72) studied a total of 2,526 subjects underwent FDG PET/CT examinations, and reported that liver SUVmax and SUVmean rise rapidly until the age of 20 and then show a slow upward trend without reaching a plateau (Figure 8).…”

Meier et al (11) noted that, the metabolic activity of the liver increases significantly with age in adults, and the overall metabolic volumetric product, a measure that takes into account an organ's volume and metabolism by multiplying the hepatic volume by the hepatic mean SUV, trend slightly upward with age.…”

“…The effect of hypothyroidism on referential background metabolic activity on 18 F-FDG PET/CT blood pool and the liver is often used as references for accurate treatment response interpretation (1)(2)(3). However, variations in biological factors such as blood glucose level and incubation time (time from FDG injection to PET/ CT scan) can affect the 18 F-FDG uptake in these regions (4)(5)(6)(7). This can be problematic when a metabolic response is defined, especially for quantitative evaluation, based on the PET Response Criteria in Solid Tumors (PERCIST).…”

“…The Deauville classification of FDG PET response, first published in 2009, is 5-point PET scoring system based on maximum standardized uptake volume (SUVmax) relative to reference tissues (mediastinal blood pool and liver), that predict the response of lymphomas to treatment ( Table 3 ) [ 4 , 49 ]. Although there is some evidence of age-related changes in metabolic activity in background tissues [ 50 ], metabolic activity in the liver and mediastinal blood pool are regarded as being relatively stable from one imaging session to the next. This system was further refined based on the outcome of an international workshop in Menton, FR in 2014 to include a wider range of lymphomas, to apply the scoring system to end-of-treatment FDG PET-CT (ePET) as well as interim PET-CT (iPET), and to introduce the concept of ΔSUVmax from baseline to iPET [ 51 ].…”
