Aggrandizing oral submucous fibrosis grading using an adjunct special stain: A pilot study

Abstract: Introduction:Oral submucous fibrosis (OSMF) is graded according to various histological factors which include the epithelial changes and the connective tissue changes. These features though could be identified in routine hematoxylin and eosin (H and E) staining; they could be better appreciated in special stains. This pilot study is an attempt to identify a single special stain that can act as an adjunct to H and E stain to help grade this potentially malignant disease.Aims and Objectives:To assess if special … Show more

“…In addition, endothelial lining of such vessels is discontinuous, which explains extravasations of RBCs discussed before (10). Moreover, these BVs demonstrate platelet aggregation and thrombosis (10,15). Because the intact endothelium prevents activation of platelets, the endothelial discontinuity may be one of the underlying reasons for platelet activation and subsequent thrombosis (18).…”

“…Normal, dilated, constricted, and obliterated blood vessels (BVs) are often encountered in OSF . The diminution in the caliber of BV first occurs in the superficial mucosa before involving deeper vessels .…”

“…Mallory's stains most effectively highlighted variations in the thick keratin layer of the stratified squamous epithelium, subepithelial edema, subepithelial hyalinization, fibrillar and homogeneous collagen, and areas of degenerating skeletal muscle bundles and hyalinization. However, it barely revealed constricted blood vessels [77].…”

“…It is worth noting that the OSF can transform to oral squamous cell carcinoma (OSCC), and that solid biopsy is needed for clinical diagnosis and treatment. In histological staging/classification, the amount and distribution of fibroblasts, collagen fibers, inflammatory cells, and blood vessels is used to determine the various stages of OSF [ 60 ]. Generally, OSF is classified into four histopathological stages ( Table 2 ) [ 61 , 62 ].…”
