Aggravation of Thiamine Deficiency by Magnesium Depletion: A Case Report

Abstract: Aggravation of thiamine deficiency by magnesium depletion. Acta Med Scand 1985; 218: 12!3-31.A patient with Crohn's disease and long-standing diarrhea resulting in a combined thiamine and magnesium deficiency is presented. Despite massive doses of thiamine i.v., the symptoms of thiamine deficiency could not be suppressed until the magnesium deficiency was corrected as well. This case report emphasizes the dependence of thiamine on magnesium for an adequate function in the body.

“…Transketolase requires TPP and magnesium as a cofactor [19]. Case studies suggest that hypomagnesaemia is a potential cause of occasional non‐response to thiamine replacement in some patients with WKS [19–21]. Animal studies confirm that magnesium deficiency interferes with the response to thiamine replacement, and the effect of magnesium deficiency appears to be due to interference with activation to transketolase [18].…”

“…Long-standing diarrhea resulted in combined thiamin and magnesium deficiency. Despite massive doses of thiamin administered intravenously, the symptoms of thiamin deficiency could not be relieved until correction of the magnesium deficiency (18). Wernicke encephalopathy, a well known result of thiamin deficiency, has been reported as being induced by magnesium deficiency (19).…”

“…Development of thiamine deficiency occurs mainly in alcoholics and in patients with malabsorption or malnutrition. Several case reports have been published describing vitamin B1 deficiency in patients with Crohn’s disease (CD) [15,16,17]. Hahn et al [15] describe a CD patient on TPN, in whom beriberi and Wernicke’s encephalopathy developed because multivitamin infusions were switched to an oral multivitamin formula.…”

“…Hahn et al [15] describe a CD patient on TPN, in whom beriberi and Wernicke’s encephalopathy developed because multivitamin infusions were switched to an oral multivitamin formula. Another case report emphasizes the dependence of thiamine on magnesium and states that if thiamine and magnesium are deficient at the same time, both should be substituted simultaneously [16]. …”
