Agminated blue nevus - Case report

Abstract: Blue nevi are benign melanocytic lesions located in the deeper reticular dermis, consequence of failure of melanocytic migration into the dermal-epidermal junction from the neural crest. Lesions are usually asymptomatic and solitary, but may present in a multiple or agminated (grouped) pattern. The agminated subtype is formed when bluish-pigmented lesions cluster together in a well-defined area. Lesions can be flat or raised. We report the case of a patient who presented multiple bluish macules (1-3 mm in diam… Show more

“…3 Despite the wide spectrum of their clinical appearances, DMs share common histological and immunophenotypical features with subtle differences. 2 From clinical point of view, these proliferations may be divided into two main categories: blue naevus (BN) presenting mainly with papulonodular lesions (Figs 1-4a) and other DMs presenting predominantly with patchy lesions (Figs 4b,5,6,7,8,9,10; Table 1). The latter DMs have been mainly classified according to the lesion's anatomic location: Mongolian spot (MS; lumbosacral region), naevus of Ota (trigeminal nerve area) and naevus of Ito (acromioclavicular region).…”

“…Interestingly, one case highlighted a superficial spreading melanoma (tumor thickness 1.8 mm, pT2aN0M0, stage IB) developing in conjunction with acquired dermal melanocytosis on a nevus spilus . Agminated BN have also been described in conjunction with EMO syndrome/Carney complex (caused by mutations in PRKAR1A ), dermatomyositis, and Darier disease …”

“…24 Agminated BN have also been described in conjunction with EMO syndrome/Carney complex (caused by mutations in PRKAR1A), 25 dermatomyositis, 26 and Darier disease. 27 Blue nevi are triggered by constitutive activating mutations in GNAQ or GNA11, the protein products of which signal through the mitogen-activated protein kinases (MAPK) pathway. The most commonly observed mutations are GNAQ p.Q209L and GNA11 p. Q209L, 1,2 representing a mutational hotspot in the genes encoding these proteins.…”

“…Blue nevi are benign melanocytic lesions located in the deeper reticular dermis, usually asymptomatic and solitary. [1] Multiple blue nevi have rarely been reported and are defined as plaque-type, agminated, or eruptive blue nevi. They are generally clustered on a cutaneous surface measuring <10 cm.…”
