The administration of electroconvulsive therapy (ECT) to children and adolescents remains an unresolved area of clinical debate for nurses. Thus, some nurses have refused to participate in the treatment of minors with ECT, invoking codes of conduct to justify their actions. Other nurses have supported the use of ECT with children and adolescents, via provision of technical assistance to medical colleagues. A cross-national comparison of ethical codes of conduct has confirmed that nurses should take decisive action in the clinical arena when the needs or rights of vulnerable minors are compromised. The provision of clinical guidelines is suggested as one method to enshrine the rights of at-risk children and adolescents.

More than 16,000 citations related to AIDS and HIV infection between May 1987 and June 1990 (38 months) were surveyed to summarize nursing research related to HIV infection; 731 nonresearch and 54 research articles, averaging about 20/month, were found. However, only 20 of the research articles addressed the five topic areas set as priority by the National Center for Nursing Research Priority Expert Panel (physiological and psychosocial aspects of care, prevention of transmission, care delivery systems, and applied ethics). There is a continued gap in the research literature related to the care aspects of HIV infection. This gap needs to be filled by nursing investigation.

A survey conducted throughout the United Kingdom examined the current provision being made to train nurses to care safely and sensitively for patients with HIV and AIDS. Problems relating to intolerance and prejudice are still commonly encountered. The study recommends that HIV and AIDS should be set into a wider context of sexuality, death and infection control rather than singled out for special attention. Nurses, both students and qualified staff, need to gain insight into their own values and prejudices if these are not to inhibit the giving of good quality care. Teaching in this subject area requires special skills and there is concern that nursing is not equipped to deal with it. The involvement of clinical specialists in educating nurses is perceived to have a strongly positive effect, helping to bridge a credibility gap and compensating for the lack of clinical experience amongst the teachers. Providing opportunities for nurses to meet with people whose lives have been profoundly affected by the virus is also a powerful form of learning. There is, however, a great need for support in this stressful area of practice.
