Airway auscultationA new method of confirming tracheal intubation

Abstract: SummaryUnrecognised accidental oesophageal intubation remains an important cause of morbidity and mortality in anaesthetic practice. We have evaluated a new method of distinguishing tracheal from oesophageal intubation using a simple adaptation of an ordinary stethoscope which is inserted into the patient end of the breathing system. We call this technique 'airway auscultation'. Characteristic sounds are heard with the stethoscope during inflation and deflation which allow the observer to diagnose the position… Show more

“…Stethoscopes have been used to detect leaks around uncuffed tracheal tubes in children [30] and in animal studies [31]. Stethoscopes have also been used as a leak detector in a tracheal tube pressure relief valve system [27], and have been investigated as a tool to detect successful tracheal intubation [32, 33]. To our knowledge, the use of stethoscopes to guide tracheal tube cuff inflation to optimal pressures has not been studied.…”

“…Previous studies have found a difference in the incidence of the sore throat between men and women. 15,16 Similarly, some studies have shown that the incidence of post operative sore throat was higher in the age group of 18-30 years (18.3%) and 30-40 years (26.2%), but it was <5% in patients aged >50 years. 17 In a study by Rahmani et al, 18 they compared the pilot balloon palpatory method with a fixed volume of 10 ml for inflating the cuff, and the result demonstrated a cuff pressure of 118.15 ± 22.15 cmH2O and 44.96 ± 21.77 cmH2O respectively.…”

“…Several tests and observations have been proposed for the diagnosis of oesophageal intubation, all of which have limitations and varying degrees of reliability [5–7]. These include the absence of carbon dioxide in the expired gas on capnograph, the oesophageal detector device and, historically, sonomatic confirmation of tracheal intubation (SCOTI) [8].…”
