Airway Injuries after One-lung Ventilation: A Comparison between Double-lumen Tube and Endobronchial Blocker

Abstract: Clinicians should be aware of an increased incidence of minor airway injuries that may impair patient satisfaction when using a double-lumen tube instead of an endobronchial blocker for one-lung ventilation.

“…Double-lumen endobronchial tubes are commonly used for lung separation and one-lung ventilation and are associated with a higher rate of laryngeal complications, such as postoperative sore throat and hoarseness, compared with single-lumen ETTs. 5,14 The incidence of postoperative sore throat and hoarseness is directly correlated with the size of the ETT. 2,15 The outer diameter of a DLT (13-14 mm for sizes 37 and 39 Fr; 12-13 mm for size 35 Fr) is larger than that of a singlelumen tracheal tube (10.7 mm for 8.0-mm internal diameter [ID]; 10.0 mm for 7.5-mm ID; 9.5 mm for 7.0-mm ID).…”

“…4 The double-lumen endobronchial tube (DLT) used for onelung ventilation is more likely to cause a higher incidence of postoperative hoarseness than a bronchial blocker inserted via a standard tracheal tube. 5 Moreover, although silicone DLTs are associated with a lower incidence of sore throat than polyvinyl chloride DLTs, 6 the latter are used routinely in the clinical setting.…”

“…In such cases, bronchial blockers are a reasonable alternative [24]. Furthermore bronchial blockers cause less postoperative sore throat and hoarseness than double-lumen tube [25,26].…”

“…Bu nedenle ÇLT'ler postoperatif boğaz ağrısı ve ses kısıklığından, trakea ve bronş rüptürüne kadar giden komplikasyonlara neden olabilirler [31,32] . BB'lere ait komplikasyonlar ise daha çok yapısal özellikleriyle ilgilidir.…”
