Airway obstruction caused by rapid enlargement of cervical lymphangioma in a five‐month‐old boy

Abstract: Key Clinical MessageCervical lymphangioma can cause airway obstruction secondary to enlargement following infection. Physicians should be aware that the airway obstruction can progress rapidly when patients with cervical lymphangioma have respiratory symptoms. Sclerotherapy for lymphangioma can cause both transient swelling and airway obstruction; thus, prophylactic and elective tracheostomy should be considered.

“…The involvement of retropharyngeal space is rare, and only few cases were reported [4][5][6]. Airway obstruction, dyspnea, and stridor in patients with lymphangioma is caused by external compression or intrinsic involvement of upper airways, which could lead to emergency tracheostomy [7].…”

“…But, the case reported by Desir [ 8 ] manifested fever, chest pain and dyspnea for a month. OK-432 is also effective, but Shimizu [ 9 ] emphasized that with the injection of OK-432, temporary swelling and pyrexia may occur. They also pointed out that sclerotherapy may take a long time to produce the desired result and induce inflammation that may cause airway obstruction due to the transient enlargement of the lymphatic cavity.…”

“…The malformations may also threaten the function of adjacent or affected tissues, organs, and as a result, appropriate treatment may be avoided [ 10 , 11 ]. Cervical lymphatic malformations can predispose an individual to complications such as haemorrhage or infection, which further affect respiratory function and endanger life [ 12 14 ].…”
