Albumin‐bilirubin grade‐based nomogram of the BCLC system for personalized prognostic prediction in hepatocellular carcinoma

Abstract: Background & Aims: The prognostic accuracy of individual hepatocellular carcinoma (HCC) patient in each Barcelona Clinic Liver Cancer (BCLC) stage is unclear. We aimed to develop and validate an albumin-bilirubin (ALBI) grade-based nomogram of BCLC to estimate survival for individual HCC patient. Methods: Between 2002 and 2016, 3690 patients with newly diagnosed HCC were prospectively enrolled and retrospectively analysed. Patients were randomly split into derivation and validation cohort by 1:1 ratio. Multiva… Show more

“…By contrast, the ALBI score is calculated using only serum albumin and bilirubin levels, both of which are incorporated in the conventional C–P score, and it does not require evaluation of prothrombin time, another parameter in the C–P classification system. Recently, the ALBI score/grade has been widely used for predicting outcomes, especially in the field of HCC 17 19 . Regarding the analysis of clinical outcomes in PBC, Fujita et al and Chan et al reported the utility of the ALBI score as a predictive marker 20 , 21 .…”

“…Nomograms provide individual predicts of future clinical outcomes by combining the effects of various variables associated with these events. The nomogram has proven to be a reliable tool for predicting the clinical survival of many types of cancers [26][27][28][29][30]. In this study, we screened 13 miRNAs, and a nomogram was then built based on the independent prognostic variables associated with patient survival.…”

“…The ALBI score was calculated according to the following equation = 0.66 × log 10 bilirubin (μmol/L) − 0.085 × albumin (g/L). ALBI grades were classified into three groups: ALBI grade 1 (score ≤−2.60), ALBI grade 2 (score >−2.60 and ≤−1.39) and ALBI grade 3 (score >−1.39) (12,14). The equation of estimated glomerular filtration rate (eGFR) was calculated based on the modification of diet in renal disease (MDRD) formula (22).…”

“…Recently, the albumin-bilirubin (ALBI) grade, consisting of only serum albumin and bilirubin, was reported a simple and objective method to assess hepatic dysfunction. The feasibility of ALBI grade in HCC has been validated by several groups (12)(13)(14)(15). However, the prognostic role of ALBI grade in HCC patients receiving dialysis has not been evaluated.…”
