Aldosterone-potassium ratio predicts primary aldosteronism subtype

Abstract: Objective: Prediction models have been developed to predict either unilateral or bilateral primary aldosteronism, and these have not been validated externally. We aimed to develop a simplified score to predict both subtypes and validate this externally. Methods: Our development cohort was taken from 165 patients who underwent adrenal vein sampling (AVS) in two Asian tertiary centres. Unilateral disease was determined using both AVS and postoperative out… Show more

“…This observational retrospective study was registered at http://ClinicalTrials.gov (NCT04428827). Baseline clinical data were obtained from patient medical records as previously described, 13 and all patients included in our study fulfilled the diagnostic criteria recommended by the Endocrine Society guidelines 14 . Details on diagnostic testing are available in Data S1.…”

“…We had developed a clinical prediction score, APR, to predict unilateral PA in our Asian cohort and validated this in a European cohort, utilizing only patients with a successful AVS 13 . A high APR was predictive of unilateral disease, with an 84.4% probability in those with APR >10, and 91.7% in those with APR >15.…”

“…Hypokalaemia, if present, was corrected with potassium supplementation, aiming for serum potassium ≥3.5mmol/L before hormone measurements. Plasma aldosterone concentration (PAC) and plasma renin activity (PRA) were determined in all patients as previously reported 12 . Patients underwent intravenous seated saline infusion test, and all patients had a post-saline infusion PAC ≥140pmol/L 6 .…”

“…In both centers, we confirmed the diagnosis of PA using the intravenous saline infusion test (SLT), and all patients had a post-SLT PAC at least 140 pmol/L. Clinical data were obtained as described in our prior study on this cohort [ 17 ] and include baseline demographics and pretreatment PAC and PRA. We excluded patients treated with adrenalectomy only and not started on medical therapy, prescribed medical therapy for less than 4 weeks before adrenalectomy, or those on medical therapy for less than 3 months.…”
