Alpha interferon for hepatitis C virus infection in haemophilic patients

Abstract: We have conducted a controlled trial in 20 haemophilic patients in which intravenous recombinant interferon alpha-2a, 3 mega units thrice weekly, was used to treat chronic hepatitis C infection. The study endpoints included complete and paritial normalization of serum ALT, and loss of serum HCV RNA as determined by polymerase chain reaction (PCR). Interferon treatment was effective, and resulted in improvement in ALT in nine (45%) and a loss of HCV RNA in five patients (26%), but a sustained normalization of A… Show more

“…The natural clearance rate of 17.4% is similar to that observed in non-haemophilia cohorts, the slightly lower level possibly reflects the preponderance of males in our study [4]. Previous data report lower rates of SVR for monotherapy in PWH compared with other HCV-infected groups [16][17][18][19] and our SVR (14.5%) is consistent with this. Combination therapy with interferon-alpha and ribavirin produces higher SVR in the non-haemophilia population with levels of 55% using pegylated interferon-alpha and ribavirin [20].…”

“…Two patients received alpha interferon from 1987 and have previously been reported 7. Alpha interferon alone was used in 53 treatment episodes, including five patients who were treated twice and one who received three courses of therapy 8. We participated in a double blind controlled trial of ribavirin versus placebo in 12 patients in 1992 and subsequently continued open label therapy in two individuals 9.…”

“…HIV coinfection is associated with more severe histological appearances suggesting that HIV accelerates progression of HCVrelated liver disease in haemophiliacs (Eyster et al, 1993;Telfer et al, 1994). The most recent studies suggest the overall sustained response rate is very low (Telfer et al, 1995;Hanley et al, 1996). Close surveillance of this group is required, and the most appropriate treatment options, including liver transplantation, should be based on the circumstances of each individual.…”
