Alterations in gut microbiota and elevated serum bilirubin in primary biliary cholangitis patients treated with ursodeoxycholic acid

Abstract: Background:The prognosis of primary bile cholangitis (PBC) is linked to gut microbiota dysbiosis. This study investigated the association between the gut microbiome and elevated total bilirubin (TB) level in PBC patients treated with ursodeoxycholic acid (UCDA). Methods: A total of 47 PBC patients with 12 months of UCDA treatment were enrolled. Patients were divided into the TB (+) (TB>1× upper limit of the normal range [ULN]; n = 20) and TB(−) (TB≤1× ULN; n = 27) groups. Stool and serum specimens were collect… Show more

“…Of the 964 patients, 342 (35.5%) had developed liver cirrhosis (LC) prior to enrollment. From this cohort, we previously reported the HLA alleles and their associations with ANA ( 4 ), gut microbiota alterations and elevated serum bilirubin ( 10 ), and the clinical characteristics of the patients with anti-gp210 ( 12 ). Here, we further explored the association between the HLA alleles and the microbiota in these patients ( Figure 1 ).…”

“…Microbiota compositions are reported to be significantly correlated with poor prognoses for patients with PBC ( 10 ). Some intestinal microflora have been associated with bile acid metabolism in PBC, and some microbes produced by short-chain fatty acids were significantly decreased in patients with PBC with inferior remission ( 6 ).…”

“…Clinical microbiota profiling also revealed the relationship between hepatic diseases and the relative abundance of Anaerostipes [ 33 , 34 ]. When compared with patients during the progression of chronic hepatitis B disease, healthy individuals had a higher distribution of Anaerostipes in their fecal microbiota [ 33 ].…”

“…The prognosis of PBC is related to the intestinal flora. Han et al (82) found that after 12 months of UCDA remedy, TB levels in PBC patients were associated with a unique gut microbiome. One of the pharmacological effects of BA sequestrant is to increase the activity of the BA receptor TGR5 in the gut, leading to the release of the downstream hormone glycoadenotide peptide -1 (GLP-1) (83,84).…”
