AGRADECIMENTOSsendo maior na primeira semana de coleta em comparação às demais. O L 10 foi de 67,0; 65,0 e 66,0dBA, respectivamente, na primeira, segunda e terceira semanas. A partir do inventário do ruído, verificou-se que maior número de pessoas circulando na UCIN, tonalidades altas da voz nas conversas, a presença de alarmes estridentes, a manipulação não cuidadosa ao fechar armários/gavetas/tampas de lixo/portas, alto fluxo da água da torneira do lavabo e quedas de objetos foram geradores de ruídos intensos. Concluiu-se que o ruído na UCIN foi intenso em todos os dias de coleta de dados, estando acima das normas técnicas e recomendações internacionais, fato também constatado em outros estudos. Recomenda-se para a redução do ruído em UCIN trabalhar de forma interdisciplinar e intersetorial, intervindo junto à equipe de saúde, clientela, ambiente físico e equipamentos.Palavras-chave: Ruído. Hospital. Neonatal. Recém-nascido. closets/drawers/lids, high flow of water in the restroom faucet and drops of objects were generators of intense noises. It was concluded that the noise at the NICU was intense in all the days of data collection, and it is above the technical standards and international recommendations, a fact, indeed, also verified in other studies. In order to accomplish the reduction of noise in a NICU, it is recommended to work in a interdisciplinary and intersectional mode, intervening on the health staff, clients, physical atmosphere and equipments.

RESUMOOs prematuros são submetidos a diversas manipulações e procedimentos durante internação em unidades neonatais, com consequências deletérias para a saúde. O objetivo do presente estudo foi descrever a manipulação a que são submetidos os prematuros durante as 24 horas em uma unidade de terapia intensiva neonatal (UTIN). Estudo observacional, descritivo, exploratório, realizado com 20 prematuros, filmados continuamente por um período de 24 horas, de setembro de 2008 a março de 2009 numa UTIN. Os prematuros foram submetidos a uma média de 768 manipulações e 1.341 procedimentos. A manipulação durou em média 2 horas e 26 minutos em um período de 24 horas. Cada manipulação agrupou uma média de 2,2 procedimentos, a maioria no turno matutino As manipulações isoladas representaram 65,6% do total de manipulações e a maioria teve duração inferior a um minuto. Conclui-se que nas 24 horas avaliadas, os prematuros foram submetidos a um excesso de manipulações na UTIN. Level of NICU Quality of Developmental Care and Neurobehavioral Performance in Very Preterm Infants

WHAT'S KNOWN ON THIS SUBJECT: Although developmental care in NICUs reduces the stress experienced by preterm infants, the actual level of developmental care may vary and little is known about how the level of developmental care relates to preterm infants' neurobehavioral performance. WHAT THIS STUDY ADDS:The study demonstrates the relationship between variations in developmental care in NICUs and the neurobehavior of preterm infants. Infants from NICUs with high-quality developmental care compared with infants from units with low quality of care evidenced a better neurobehavioral profile.abstract OBJECTIVE: To examine the relation between the neurobehavior of very preterm infants and the level of NICU quality of developmental care. METHODS:The neurobehavior of 178 very preterm infants (gestational age #29 weeks and/or birth weight #1500 g) from 25 NICUs participating in a large multicenter, longitudinal study (Neonatal Adequate Care for Quality of Life, NEO-ACQUA) was examined with a standardized neurobehavioral assessment, the NICU Network Neurobehavioral Scale (NNNS). A questionnaire, the NEO-ACQUA Quality of Care Checklist was used to evaluate the level of developmental care in each of the NICUs. A factor analyses applied to NEO-ACQUA Quality of Care Checklist produced 2 main factors: (1) the infant-centered care (ICC) index, which measures parents' involvement in the care of their infant and other developmentally oriented care interventions, and (2) the infant pain management (IPM) index, which measures the NICU approach to and the procedures used for reducing infant pain. The relations between NNNS neurobehavioral scores and the 2 indexes were evaluated. RESULTS:Infants from NICUs with high scores on the ICC evidenced higher attention and regulation, less excitability and hypotonicity, and lower stress/abstinence NNNS scores than infants from low-care units. Infants from NICUs with high scores on the IPM evidenced higher attention and arousal, lower lethargy and nonoptimal reflexes NNNS scores than preterm infants from low-scoring NICUs.CONCLUSIONS: Very preterm infant neurobehavior was associated with higher levels of developmental care both in ICC and in IPM, suggesting that these practices support better neurobehavioral stability.
