Alveolar socket preservation with demineralised bovine bone mineral and a collagen matrix

Abstract: PurposeThe aim of the present study was to evaluate the healing of post-extraction sockets following alveolar ridge preservation clinically, radiologically, and histologically.MethodsOverall, 7 extraction sockets in 7 patients were grafted with demineralised bovine bone mineral and covered with a porcine-derived non-crosslinked collagen matrix (CM). Soft tissue healing was clinically evaluated on the basis of a specific healing index. Horizontal and vertical ridge dimensional changes were assessed clinically a… Show more

“…Although fully epithelialized, the soft tissue invagination observed at 9/10 cases after 8-10 weeks of healing indicates that the mucosa had not fully healed. This is consistent with a clinical study that used a soft tissue healing index to evaluate the healing extraction sockets with CM placed over DBBMC in seven patients (Maiorana et al, 2017). At 4 weeks, there were no sites that had completely healed, whereas at 8 weeks, 5/7 sites had healed completely.…”

“…In the present study, CM was placed over the DBBMC to provide an initial barrier and to subsequently promote soft tissue closure over the socket. The CM undergoes a substitution process and is replaced with a vascularized collagen-rich connective tissue (Maiorana et al, 2017). Although fully epithelialized, the soft tissue invagination observed at 9/10 cases after 8-10 weeks of healing indicates that the mucosa had not fully healed.…”

“…Alveolar ridge preservation (ARP) is considered to be a valid therapeutic option since it significantly reduces the dimensional changes of the ridge post‐extraction (Avila‐Ortiz, Elangovan, Kramer, Blanchette, & Dawson, ; Choi et al, ; Maiorana et al, ). Previous data suggested that ARP may prevent the pneumatization process and reduce the need for subsequent sinus augmentation at implant placement in the posterior region (Levi, Halperin‐Sternfeld, Horwitz, Zigdon‐Giladi, & Machtei, ; Rasperini, Canullo, Dellavia, Pellegrini, & Simion, ).…”

“…Bone grafts will also act as an osteoconductive material to stabilize blood clot and advance bone regeneration by providing a scaffold, thus enhancing the migration of osteoprogenitor cells. [12][13][14] Our patient had received xenograft bone Osteo-LEMB to restore the defective site since the technique was well accepted by him and does not contravene his faith and beliefs. Active and fast proliferation of the soft tissue from the buccal, lingual and palatal aspects may interfere with the bone ingrowth from approximal bone surfaces which result in a fibrous connective tissue core.…”
