Amplitude-oriented exercise in Parkinson’s disease: a randomized study comparing LSVT-BIG and a short training protocol

Abstract: LSVT-BIG is an exercise for patients with Parkinson's disease (PD) comprising of 16 1-h sessions within 4 weeks. LSVT-BIG was compared with a 2-week short protocol (AOT-SP) consisting of 10 sessions with identical exercises in 42 patients with PD. UPDRS-III-score was reduced by -6.6 in LSVT-BIG and -5.7 in AOT-SP at follow-up after 16 weeks (p < 0.001). Measures of motor performance were equally improved by LSVT-BIG and AOT-SP but high-intensity LSVT-BIG was more effective to obtain patient-perceived benefit.

“…In a recent work [45] a lower reduction was observed after a shorter duration (2 weeks) version of the same LSVT protocol (a total of 8 h of treatment). Our findings are consistent with the abovementioned ones taking into account that our training protocol duration is slightly higher than [45] but lower than [27] and [44].…”

“…This program resulted in better patient‐reported health effects than a shortened program of two weeks. Surprisingly, effects on motor function were equal, indicating that patient‐reported outcomes are not identical to more objective measures . This observation once again underscores the challenge of selecting the proper outcome for trials in this field.…”

“…The program is characterized by increasing complexity, high intensity, and multiple repetitions. It is carried out in a 4‐week program of four training sessions per week . This program resulted in better patient‐reported health effects than a shortened program of two weeks.…”

“…In addition, specific exercise programs have been developed and studied recently. Potentially, effective programs are an Intensive Rehabilitation Program (4 weeks, 5 times a week, combined types of exercise) [78,79], and the Lee Silverman Voice Treatment -BIG program (high amplitude movements, sensory recalibration and self-cueing) [80]. Other types of exercise for which evidence is increasing are Tai Chi [81][82][83], hydrotherapy [84], boxing [85] and dancing [86][87][88].…”
