An abnormal pancreatico-choledocho-ductal junction in cases of biliary tract carcinoma

Abstract: A histopathologic study on how the common bile duct and main pancreatic duct open into the duodenum was performed on 72 autopsied cases of biliary tract carcinoma. Type IIIb which was considered to be an abnormal pancreatico–choledocho–ductal junction was identified in 8 of 34 cases in common bile duct carcinoma and in 4 of 24 cases in gallbladder carcinoma, while none of the control cases belonging to Type IIIb. In cases of Type IIIb, reflux of pancreatic juice may occur into the bile duct and produce the rep… Show more

“…Early-stage GB cancer is often detected in resected specimens after cholecystectomy for benign diseases [1,2] . Moreover, GB cancer is sometimes detected in association with pancreaticobiliary maljunction (PBM), which is the union of the pancreatic and biliary ducts at a site outside the duodenal wall [3] . Some previous studies have described spontaneous necrosis of parathyroid and adrenal gland tumors [4][5][6][7] .…”

“…The main pancreatic duct and the common bile duct open into the duodenum, where they frequently form a com mon channel, the incidence of which is reported to be from 55% [1] to 82% [2] . The length of the common channel ranges from 1 mm to 12 mm, with an average length of 4.4 mm [3] .…”

“…2,3 A number of EBDCs arise in bile ducts with an abnormal pancreatico-biliary duct junction, and their tumorigenesis is considered to be associated with inflow of pancreatic juice into the bile duct. [4][5][6][7] However, more common mechanisms for the tumorigenesis and the reasons for the biological aggressiveness of EBDCs have not yet been sufficiently elucidated, and further investigations are required.…”
