An alternative approach to adjust the iron status for inflammation in population: an exploratory study

Abstract: Background: Iron deficiency (ID), one of the major micronutrient deficiencies globally, is challenging to measure. This is due to the dual character of ferritin which measures both the ID and concurrently affected by inflammation and infection. To address this, inflammation biomarkers—e.g., CRP (C-Reactive Protein) and AGP (Alpha-1- Acetylated Glycoprotein) are used through various standard methods to report inflammation-adjusted ID. However, there are problems with these methods and their underlying principle… Show more

“…The other possible reason for hepcidin‐induced inhibition of iron absorption is the presence of inflammation and infection. However, the setting is non‐endemic for malaria, and the presence of common infection and/or inflammation as measured by the CRP > 5 mg/L was 4.9% (school children) and 9.5% in non‐pregnant non‐lactating women [29] which is an acceptable level for a developing country. Moreover, the potential effect of inflammation was adjusted to report the inflammation‐adjusted ferritin status in the present study.…”

“…Since, inflammation affects adversely the concentration of haemoglobin (37,38) , ferritin which is closely associated with haemoglobin is adjusted for inflammation by excluding the cases of haemoglobin from analysis when C-reactive protein (CRP) is >1 mg/L (24) .…”

“…Ferritin and CRP is measured by Sandwich ELISA at the Nutritional Biochemistry Laboratory of the International Centre of Diarrhoeal Diseases Research, Bangladesh (ICDDR,B). Ferritin is adjusted for inflammation following the method shown by Rahman et al (24) . Groundwater iron concentration is measured by the inductively coupled plasma atomic emission spectrophotometry (ICP-AES) (27) .…”
