An Analysis of Patient Safety Incident Reports Associated with Electronic Health Record Interoperability

Abstract: Background: With the widespread use of electronic health records (EHRs) for many clinical tasks, interoperability with other health information technology (health IT) is critical for the effective delivery of care. While it is generally recognized that poor interoperability negatively impacts patient care, little is known about the specific patient safety implications. Understanding the patient safety implications will help prioritize interoperability efforts around architectures and standards. Objectives: Our… Show more

“…One of our greatest priorities during this transition toward a digital health system should be the improvement of interoperability between existing technologies. For example, incompatibility of patient electronic medical records between hospitals, pharmacies, laboratories, and other health services remains a major challenge in health information technology (Adams et al, 2017). The ability to integrate electronic medical records with m-health smartphone applications will be conducive to more efficient patient-physician telemedicine sessions and greater patient self-efficacy.…”

“…4 In healthcare systems that make significant use of IT, considerable numbers of patient safety incidents and adverse drug events have been linked to interoperability-related challenges. 5,6 Plans to improve interoperability have been part of multiple recent NHS documents, including the NHS Long Term Plan, 7 the Wachter Review, 8 the Topol Review 9 and the Five Year Forward View. 10…”

“…For example, interoperability and data exchange failure issues may be more common with laboratory-and radiology-related errors. 45 If the taxonomy is not suitable for these other types of events, the validation process would ideally provide insights into how the taxonomy can be modified to the best classify those events.…”
