An assessment of serum leptin levels in patients with chronic viral hepatitis: a prospective study

Abstract: Background: The role of leptin in the course of liver disease due to chronic viral hepatitis (CVH) remains controversial. Our aims were to investigate the relationship between serum leptin concentrations and the severity of liver disease in a cohort of subjects with HBeAg negative chronic hepatitis B (CHB) and C (CHC) and to analyze the effect of body composition, the leptin system and insulin resistance together with viral factors on virologic response to antiviral treatment.

“…We found that serum leptin levels were significantly associated with fibrosis in patients with chronic HCV-4 infection. This is in agreement with many previous studies [23,29,30] . In contrast, several other studies have shown no association between serum leptin and fibrosis in HCV infection [31][32][33] .…”

“…Serum leptin was found to be higher in patients with cirrhosis during the course of chronic viral hepatitis (20 due to chronic hepatitis B and 15 due to CHC) (116). Moreover, another study showed a significant association between serum leptin and fibrosis stage in HCV-and hepatitis B virus-infected patients (117).…”

“…Although a high incidence of hyperleptinemia has been observed in HCV infected patients with liver steatosis [22], the underlying mechanism promoting this effect remains undefined. Leptin may increase insulin resistance and fatty acid concentrations in the liver, leading to enhanced lipid peroxidation and promoting steatosis.…”
