In this report, the authors examined the characteristic features of morphology and molecular biology of Ki-67, p53, Bcl-2, and cyclooxygenase-2 (Cox-2) immunocytochemistry in low-grade endometrioid endometrial carcinoma (LG-ENEC) and disordered proliferative (DP)/benign hyperplastic (BH) endometrium. We carried out a prospective study by collecting endometrial imprints from freshly resected uteri over a 20-month period and finally 104 patients were evaluated with endometrial cytology. We focused on LG-ENECs, as well as on BH endometrium and its precursor lesion, DP endometrium, firstly because of the overlapping cytomorphology of these pathologic entities and secondly because of the lack of agreement in the differential diagnosis of atypical hyperplasia from complex hyperplasia and well-differentiated endometrial carcinoma, even in curettage specimens. Ki-67 expression of LG-ENEC showed predominance in comparison with DP/BH endometrium. Furthermore, high levels of Bcl-2 (>50%) were expressed only in DP/BH endometrium. DP/BH endometrium was negative for p53 marker, except from two cases of BH endometrium. Cox-2 expression ≥50% was found only in LG-ENECs. Using Ki-67, Bcl-2, p53, and Cox-2 markers, we managed to distinguish fully DP/BH endometrium from LG-ENEC. Higher Ki-67%/Bcl-2% rate and also higher Cox-2 expression were found in LG-ENEC cases with FIGO stage ≥ IC, than in cases with FIGO stage < IC. The immunocytochemical findings from a combination of Ki-67, p53, Bcl-2, and Cox-2, may differentiate LG-ENEC from DP/BH endometrium with overlapping cytomorphology. Immunocytochemistry appeared to be useful also for the correlation between LG-ENEC and FIGO stage.

Background: Abnormal uterine bleeding (AUB) is associated with a wide variety of endometrium. Endometrial interpretation is valuable not only to find the etiology of AUB but also to rule out any organic cause for abnormal uterine bleeding. Fractional curettage is one of the most reliable and commonly performed diagnostic techniques used in the investigation of AUB. To evaluate the efficacy of endometrial aspiration cytology in the diagnosis of abnormal uterine bleeding by correlating it with histopathology.Methods: This comparative study to be conducted in100 patients attending antenatal clinic at Department of OBG, Tirunelveli Government medical college hospital from January 2018– July 2019. (17) months. Specific investigations like abdominal ultrasonography, thyroid profile, platelet count, bleeding time, clotting time, prothrombin time, etc were done whenever required. Informed consent was taken from all the study patients. Prior to Fractional curettage, endometrial aspiration was done using plastic disposable pipelines of 3 mm diameter.Results: The sensitivity of the cytology in detecting carcinoma of the endometrium was 83.3%. The specificity of cytology is 100%. Hence the test is highly specific in diagnosis. The positive predictive value is also 100% and the negative predictive value is 99%.Conclusions: It can also detect a few of the endometrial pathologies like endometrial hyperplasia, atrophy, and atypia. It is office Procedure. reduction in associated surgical risks, expense, post-operative pain, and need for operative anesthesia as seen in fractional curettage.

Endometrial cytology has been studied for more than 25 years, and a variety of cytologic devices have been developed for direct sampling of the endometrium. The quality of endometrial samples procured by various devices is markedly different and greatly affects the diagnostic accuracy. A new endometrial sampling device, the IUMC Endometrial Sampler, was developed at the Indiana University Medical Center and approved by the Food and Drug Administration for general medical use. This device is intended for the early detection of endometrial carcinoma and its precursors. It can be used to monitor the endometrial condition of patients receiving estrogen replacement therapy or tamoxifen. It is also useful for the procurement of uncontaminated endometrial samples for microbiologic studies from patients with suspected endometritis. It has the potential to be used for endometrial dating for patients with infertility disorders. In our clinical trials and sampling tests using hysterectomy specimens, adequate and representative endometrial samples without contamination from endocervix and vagina were consistently obtained by this device. The procedure of endometrial sampling using this device and the preparation techniques for endometrial brushing specimens are discussed and illustrated. Diagn.
