Traditional Indian medical practices (Ayurveda, Siddha, Unani, and homeopathy) are a vast reservoir of knowledge about medicinal plants. The promising pharmacological properties of these plants have paved the way for developing therapy against novel Coronavirus (CoV) infection. The current review will summarize published works of literature on the effects of traditional Indian medicinal plants against acute respiratory infection (COVID‐19, SARS, Influenza, and Respiratory syncytial virus infection) and registered clinical trials of traditional Indian herbal medicines in COVID‐19. The current study aims to comprehensively evaluate the data of traditional Indian medicinal plants to warrant their use in COVID‐19 management. PubMed, Embase, and Cochrane databases were searched along with different clinical trial databases. A total of 22 relevant traditional Indian medicinal plants (35 relevant studies) were included in the current study having potential antiviral properties against virus‐induced respiratory illness along with promising immunomodulatory and thrombolytic properties. Further, 36 randomized and nonrandomized registered clinical trials were also included that were aimed at evaluating the efficacy of herbal plants or their formulations in COVID‐19 management. The antiviral, immunomodulatory, and thrombolytic activities of the traditional Indian medicinal plants laid down a strong rationale for their use in developing therapies against SARS‐CoV‐2 infection. The study identified some important potential traditional Indian medicinal herbs such as Ocimum tenuiflorum, Tinospora cordifolia, Achyranthes bidentata, Cinnamomum cassia, Cydonia oblonga, Embelin ribes, Justicia adhatoda, Momordica charantia, Withania somnifera, Zingiber officinale, Camphor, and Kabusura kudineer, which could be used in therapeutic strategies against SARS‐CoV‐2 infection.

Coronavirus disease 2019 is a worldwide pandemic resulting in a severe acute respiratory syndrome. Remdesivir is the only FDA-approved drug for hospitalized patients older than age 12. It shows the necessity of finding new therapeutic strategies. Functional foods (FFs) could have co-therapeutic and protective effects against COVID-19 infection. Traditional Persian medicine (TPM), one of the safest and most popular schools of medicine for hundreds of years, has recommended potential FF candidates to manage such a global pandemic. To reveal the potential of TPM in terms of antitussive FFs, traditional Persian pharmacopoeia “Qarabadin-e-Salehi” was searched using the keywords “Soaal” and “Sorfeh.” Also, a search of MEDLINE, PubMed Central, Google Scholar, and Science Direct was performed for the relevant literature published from the inception up to March 2021. A combination of search terms including “cough, antitussive, antioxidant, anti-inflammation, antiviral, COVID-19, mucoactive, mucolytic, expectorant, and mucoregulatory” was also applied. The potential mechanism of action in SARS-CoV-2 infection was discussed. Twelve TPM FFs were found including Laooqs, Morabbas, a Saviq, a soup, and a syrup. They are combinations of two to seven ingredients. Natural compounds of mentioned formulations have the main pharmacological mechanisms including antiviral, anti-inflammatory, antioxidant, antihistamine, bronchodilator, immunomodulatory, and mucoactive effects as well as central or peripheral antitussive activities. FFs are cost-effective, easily accessible, and safe options for both treatment and prevention of COVID-19. They might have positive psychological effects along with their pharmacological effects and nutritional virtues. They could also manage persistent respiratory discomforts after recovery from COVID-19.

BACKGROUND AND OBJECTIVE: Tiryaq-i-Afayee, a polyherbal formulation of Unani medicine used in the prevention and treatment of various wabāi amrād (epidemic/ pandemic diseases) since olden days. This review study has been carried out to explore the therapeutic efficacy of this formulation in the light of scientific studies. MATERIALS & METHODS: A total of 74 literature comprising classical Unani texts from the period of 9th – 20th century AD, and published research articles in various reputed journals from 1999 – 2020 AD were reviewed regarding immunomodulatory, antiviral, antiinfluenza, antioxidant, antitussive, antimicrobial effects of Tiryaq-iAfayee, as a whole preparation and its individual ingredients. RESULTS:Tiryaq-i-Afayee contains Sibr Zard (Aloe vera) (2 parts), Murr Makki (Commiphora myrrha) (1 part) and Zafran (Crocus sativus) (1 part). It is prepared in the form of pill, and 2 g is used in alternate day for prevention and management of epidemic diseases. All the three ingredients of this compound formulation belong to plants and individually used for various medicinal purposes. DISCUSSION: A study revealed that Tiryaq-i-Afayee possesses significant immunomodulatory effect in a small group of immuno compromised persons. An ethanol extract of Aloe vera exhibited antiinfluenza effect against influenza A virus due to presence of aloe-emodine (anthraquinone glycoside). Certain studies have reported that Commiphora myrrha possesses significant antioxidant and antimicrobial effects against Staphylococcus aureus and Pseudomonas aeruginosa. Another study has revealed that Saffron extract exhibited promising antiviral activity due to presence of crocin and picrocrocin. CONCLUSION: The aforementioned classical and modern conventional materials have given plentiful information regarding use of Tiryaq-i-Afayee in the prevention of epidemic/ pandemic diseases.
