An instructive case of amlodipine‐induced reversible granulomatous CD30+T‐cell pseudolymphoma

Abstract: We report an unusual case of drug-associated granulomatous CD30 + T-cell pseudolymphoma secondary to amlodipine. A 55-year-old Chinese man presented with a 6-month eruption of disseminated erythematous dermal papulonodules and annular infiltrated plaques over his neck and limbs symmetrically. Histopathology revealed a perivascular and interstitial infiltrate of histiocytes, eosinophils and morphologically normal lymphocytes associated with CD30 expression. The eruption improved rapidly after discontinuation of… Show more

“…The lowest prevalence of CD30‐positive reports between drug classes was observed among monoclonal antibodies (37.5%) ( p ‐value <0.001). Of 25 cases with T‐cell receptor rearrangement, 21 (84.0%) of them were polyclonal and four (16.0%) were monoclonal 30–33 . Two cases of monoclonal CPL have been reported after using amlodipine, and cases of phenytoin and metoprolol/hydrochlorothiazide‐induced CPL have been found.…”

“…Inflammatory drug eruptions have been sometimes reported to contain a large amount of CD30-positive cells. 88–93 Mainly antiepileptic drugs have been reported to have such findings. 79 Some of these infiltrates have shown an intralymphatic distribution, mimicking an intravascular lymphoma.…”
