ObjectiveWe describe the characteristics of patients with high medical costs by matching specific annual medical examination results and medical claim data. Clarifying the relationships between examination items and high medical costs allows the screening of high-risk persons.DesignA cross-sectional study.SubjectsSubjects were persons insured by national health insurance in Hiroshima City, Hiroshima Prefecture, from April 2016 to March 2017. To identify true heart failure (HF) patients, the disease name listed in the medical claim data was compared with drugs prescribed for HF, with extraction of only subjects whose comparative data matched.Data collection and analysisThe specific health examination includes a questionnaire on areas such as lifestyle habits, anthropometry, blood pressure, blood tests and urine tests. The percentage of the total medical costs related to the medical care of subjects with HF was described using Pareto analysis. For specific health examination items, we compared the high-cost and low-cost groups. The normality and homoscedasticity of each variable was checked and Student’s t-tests and χ² tests were applied. Finally, multiple logistic regression analysis was used to detect factors in the health examination items related to high medical costs.ResultsPareto analysis showed that 80% of all medical costs were paid by 30% of the HF patient population. The fees for cardiovascular surgery accounted for 54% of the total surgical cost, 64% of which included preventable diseases. Levels of creatinine (Cr) and γ-glutamyl transpeptidase (γ-GTP) and a history of smoking were found to be related to high medical costs.ConclusionAnalysis of specific health examination results for HF patients revealed the association between high medical costs, γ-GTP, Cr, and smoking. These results can thus serve as a reference for screening persons at high risk of HF and help prevent the exacerbation of HF.

Sustainable development is an important United Nations agenda, and the determination of which of the sustainable development goals (SDGs) should be prioritized is left up to each participating nation. Stakeholder engagement including all members of society can engender a nationally representative priority SDG set. This research investigates inclusive sustainable development which incorporates an approach to stakeholder engagement. The study assesses precedential scholarship of stakeholder engagement and sustainability evaluation, identifying a gap in terms of the inclusion of householder perceived importance of lifestyle related factors in sustainable development policy making and evaluation. Utilizing a case study of the aging, shrinking population of Japan, a representative, national survey of householders is undertaken, demonstrating that lifestyle relevant factors of sustainability approximate jurisdictionally important United Nations SDGs and help to identify priority targets within these goals. Perceived importance variety across household generations is also identified, providing guidance for policy makers in terms of age-relevant policy making and jurisdictionally important cultural factors. The framework proposed has applications in the case-study nation and is also readily applicable to other jurisdictions and for use in comparative studies.

There is an increase in the number of patients with osteoporotic fractures due to the aging population in Korea. This study investigated the burden of osteoporotic fractures including hip, spine, and wrist fractures in the Korean population by estimating disability-adjusted life years (DALYs). The DALY for a given condition in a population captures years of life lost due to premature death and years of life lived with a disability and its severity and duration. To calculate DALYs from all relevant data collected for the 3 conditions, we used a DALY calculation template provided by the World Health Organization in 2014. DALYs per 100 000 for vertebral fractures (3168) were higher than those of hip fractures (2496) in women. Wrist fractures (1038) had the least burden, and the difference between men and women was the lowest. The aging population is expected to increase the burden of osteoporosis.
