An outbreak of acute delirium from exposure to the synthetic cannabinoid AB-CHMINACA

Abstract: A coordinated response and collaboration between law enforcement, the local public health, emergency medical services and Health Center staff, were all key interventions in preventing a more substantial public health outbreak resulting from use of a novel synthetic cannabinoid compound. Real time collaborations between toxicology laboratories, suppliers of analytical standards and the public health system may be useful in the face of future novel chemical exposures.

“…An EC 50 value of 3.45 nM (1.96–6.14) was determined for compound 14 (Table ), which is in agreement with the low EC 50 value of 7.4 ± 1.5 nM reported by Wiley et al , which was determined by the binding of a slowly hydrolysable GTP analog to the Gα‐subunit using [ 35 S]GTPγS turnover assay, instead of the β‐arr2 recruitment assay used in this study. Hydrolysis metabolite 16 has been identified as a hydrolysis product of parent compounds 14 and 15 in a number of studies in urine and hair of abusers . Metabolite 16 clearly retains activity (EC 50 = 155 nM), although with a 45‐fold impaired potency when compared to the parent compounds (Table ).…”

“…Because of this, cases of acute poisoning, accidental overdose, and unforeseen adverse reactions associated with NPS use have become an increasing problem at emergency departments (EDs) and intensive care units (ICUs) . To study the occurrence and health hazards of newly emerging drugs of abuse in Sweden, the STRIDA project was started in 2010 .…”

“…This diagnosis is also recognized by the National Association of Medical Examiners and used by medical examiners. Excited delirium is often associated with cocaine toxicity and can be associated with synthetic cannabinoids and bath salts [5456]. There is no standardized well accepted treatment approach to these patients.…”

“…Prolactin levels are not increased in these patients. Seizures could develop in subjects without known epilepsy secondary to drug toxicity, and their detection during sustained physical confrontations, especially if akinetic, might be quite difficult [54, 55]. …”
