An Unusual Case of Lower Extremity Clear Cell Hidradenocarcinoma

Abstract: Hidradenocarcinoma (HC) is a rare malignant sweat gland tumor with metastatic potential primarily located in the head, neck, and trunk. We present an unusual case of a large lower extremity Clear Cell HC managed with surgical resection and adjuvant locoregional radiation after excluding lymph node involvement.

“…q CONCLUSION L abbaridia et al (20) state that, although HC is a rare eccrine gland tumour, it is nevertheless an aggressive one, and there are no current evidence-based guidelines. However, using sentinel lymph node biopsy, radiotherapy (RT) and chemotherapy (CHT) is still a controversial topic in HC cases (21). As shown by the present report, hidradenoma is a benign tumour, which is nevertheless difficult to differentiate from hidradenocarcinoma.…”

“…However, approximately 66% of patients present distant extension of the tumor (39% to lymph nodes and 28% to other distant organs that include bone, lung, pleura, skin, and among others), and the local recurrence rate of this type of tumor is high (up to 50%). Therefore, surgical treatment of the tumor is currently combined with regional lymphadenectomy whenever possible if there is regional lymph node involvement, and even prophylactically [ 2 , 3 , 4 , 8 , 9 , 10 ].…”

“…Sentinel node biopsy can be useful in the early diagnosis of lymph node metastases in the subclinical phase in those cases with lesions with data suggesting a high risk of malignancy, such as invasion more than 2 mm in depth, perineural invasion, and histological dedifferentiation [ 2 , 8 , 9 ].…”

“…Because it is so rare, the treatment protocols used are a subject of debate. It has local recurrence rates up to 50% and metastasis rates of around 60% despite wide excision, which is the generally accepted treatment method (17,18,19). As dermal lymphatic invasion, nerve sheath involvement, deep structure infiltration, positive resection margins, highly anaplastic morphology, and extracapsular lymph node spread are associated with high recurrence rates, these patients have been recommended adjuvant radiotherapy (20,21).…”
