An Unusual Case Of Talonavicular Dislocation With Associated Ipsilateral Foot Fractures

Abstract: We describe a previously unreported form of a talonavicular dislocation complicated by navicular, talar neck and open calcaneal fractures. Prompt recognition and rapid reduction of fractures and dislocations gives the best possible outcome. The presence of serious distracting injuries can alter priorities, however the “lesser” injuries often lead to long term morbidity after recovery from the life-threatening injuries.

“…Associated CC joint injuries are common leading to significant long term disability [10,23]. The incidence of associated injuries in the present study is comparable to previous reports of navicular and other midfoot injuries [10,13,23,25,28,29,34,35,43,46,[49][50][51][52]. Patients suffering from more complex foot injuries, represented by a higher number of associated injuries, were more prone to long term disability.…”

“…It is reflected by the fact that a PubMed search with the key words 'ipsilateral talus and calcaneum fractures' gave just 6 results with only 3 being relevant and with further manual search we were able to find one more case report with similar foot injury. 3 In a study by Seybold et al, 4 the authors reported 11 patients of whom nine had extra-articular calcaneal fractures resulting from relatively low-energy trauma. In their case series, all injuries were closed and it was concluded that most patients had a favorable clinical outcome.…”

“…Biomechanical studies have shown that residual displacements as little as 2 mm of the talar neck altered the contact characteristics of the subtalar joint. 10 Forefoot adduction, calcaneal internal rotation, and loss of subtalar motion may be associated with persistent Subtalar arthrodesis (4), All excellent or None ORIF of both fractures (1), good BKA (1), immobilization (3) Newton et al 3 …”

“…Ankylosis and equino-varus deformity can occur with incomplete talonavicular reduction. Cases of isolated mid-tarsal dislocation in medial, lateral, or plantar directions have been reported 6 . These injuries are due to complex multidirectional forces.…”
