Anabolic steroids affect human periodontal health and microbiota

Abstract: Data on the influence of AAS on subgingival periodontopathogens and disease progression are scarce. Higher proportions of specific periodontopathogens are plausible in AAS users. AAS users had a higher prevalence of severe periodontitis, gingival inflammation, and clinical attachment loss. Men taking AAS are at greater risk of periodontitis and specific periodontopathogen infection.

“…In previous studies, high serum testosterone levels were positively associated with increased risk for both advanced hepatic fibrosis and advanced hepatic inflammatory activity in hepatitis C virus–infected U.S. male veterans 32 . Additionally, men under anabolic androgenic abuse, and thus showing high testosterone levels, presented with more severe forms of periodontitis due to the modulation of microbiota 18 . In the current study, both high TT and CBT are associated with increased presence and severity of testosterone after adjustment for estradiol.…”

“…High testosterone, on the other hand, has been linked to severe forms of periodontitis and a high prevalence of periodontopathogenic microorganisms as well as Candida spp. in men using androgenic anabolic steroids 18 …”

“…One possible explanation for these findings is that testosterone possesses immunosuppressive properties, as demonstrated by the suppressive effect on leukocyte count on orchiectomized mice and young male rats [38, 39]. Along those same lines, a recently published paper demonstrated that men using anabolic androgenic steroids present with more severe forms of periodontitis and higher prevalence of periodontopathogenic microorganisms, including some species of Candida [40]. We demonstrated that Low testosterone upregulates IL-1β production only at the site of inflammation (and not systemically), but it had no impact on IL-6 either locally or systemically.…”

“…15. subjects. Among the local and/or systemic factors associated with the higher distribution of NAC species in the oral cavity, we could mention the use of prosthetic devices [11]; being elderly, [15] elderly with low body mass index [16]; being an oncological patient undergoing chemotherapy or radiotherapy [17,18]; being a diabetic patient with poor metabolic control [19], being a man using anabolic androgenic steroids [20] or a woman using oral contraceptives [21].…”
